Grace Care Consulting, LLC is looking for a qualified and experienced full-time RN who is flexible, adaptable and, above all, patient in all types of situations. In this position, the selected candidate will do a combination of field and remote work. A portion of the time will be spent in member homes, completing level of care assessments. The other part of the time will be spent completing documentation, making follow-up calls, and abiding by processes (utilizing remote home office). RN's must be located within 30 miles of the designated coverage territory. We are currently hiring for the Boston area.
This is a FULL-TIME temporary position with the potential for permanent placement.
MUST BE AVAILABLE TO WORK Monday through Friday 8:30 a.m. - 5 p.m. EST.
No weekends.
No holidays.

Requirements:
RN with an unrestricted nursing license in the state of Massachusetts.
Must live within 30 miles of designated coverage territory.
The ability to drive in designated territory to visit members in their homes and/or other settings, including community centers, hospitals, or providers' offices.
Experience working with MS Office including Excel, Outlook, Teams, OneNote and SharePoint.
STRONG technical skills are a MUST.
Preferred Qualifications:
MUST be able to motivate and direct self to ensure caseload is supported at the highest level.
Bilingual in English and Cantonese/Mandarin
10 years' experience working in a health care role
5 years’ experience as a case manager in a managed care or home health care setting
A background in managing populations with complex medical or behavioral needs
Experience with electronic medical record systems
Primary Responsibilities:
Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate and least restrictive level of care.
Identify and initiate referrals for social service programs, including financial, psychosocial, community and state-supportive services.
Manage the care plan throughout the continuum of care as a single point of contact.
Communicate the required health-related information with all stakeholders to ensure quality coordinated care and services are provided expeditiously to all members.
Advocate for members and families as needed to ensure the patient’s needs and choices are fully represented and supported by the health care team.
