Responsibilities and Duties

Provide in home physical therapy to clients
Evaluate the client's rehabilitative status and participate in the development of the total plan of care
Initiate physical therapy program and instruct other personnel in certain phases of physical therapy with which they may work with a client
Prepare and submit clinical notes and progress summaries based on the attainment of goals

Qualifications and Skills

Possesses a degree from a baccalaureate or masters program in physical therapy or a graduate of an accredited physical therapy assistant program with either an Associate’s degree in science or applied science that is approved by American Physical Therapy Association, Inc.
Valid license to practice physical therapy within the state of Massachusetts
Home care experience preferred (not required)
Must be a licensed driver with an automobile that is insured in accordance with state requirement
Bilingual encouraged to apply
Ability to successfully complete a background check
