Preferred Transitions Home Health Care is looking to fill the position of a Weekend (Friday, Saturday, Sunday OR Saturday Sunday, Monday), Start of Care Nurse.
RN with Home Care experience only
This job would entail:
Working 24 hours per week
Working weekends
Rates: competitive rates/salary
Range : $55,000 - $75,000 per year based upon experience
Responsibilities:
- Assessment of patient and conditions as per homecare guidelines
- Collaborate with physicians and nurses to devise individualized care plans for patients
- Perform Start of Care visits as need for patients
